صفحه 423مقدمهتعیین ویژگی های بالینی افراد دچار اختلال تحمل گلوگز ایزوله (Isolated IGT) که به صورت گلوکز ناشتای سرم (FPG) کمتر از 100 میلی گرم بر دسی لیتر و گلوکز 2 ساعته ی سرم (2h-PG) بین 140 و 200 میلی گرم بر دسی لیتر تعریف می شود.مواد و روش هادر این مطالعه ی مقطعی، داده ها و یافته های آزمون تحمل گلوکز خوراکی (OGTT) مربوط به 8212 شرکت کننده ی مطالعه ی قند و لیپید تهران (4742 زن و 4370 مرد) با سن مساوی یا بیشتر 20 سال که مبتلا به دیابت بودند ارزیابی شد. مدل های رگرسیون لوجیستیک برای پیش بینی ابتلا به IGT ایزوله در میان 7012 فرد دارای FPG کمتر از 100 میلی گرم بر دسی لیتر ساخته شد.یافته هاشیوع کلی IGT، 6/13% (تعداد 1120 نفر) بود که از این میان 6/59% (668 نفر) دچار IGT ایزوله بودند. در مدل بالینی، نسبت های شانس تطبیق داده شده برای ابتلا به IGT ایزوله، برای سن بالای 40 (6/2)، فشار خون بالا (9/1)، اضافه وزن (7/1)، یا چاقی (2/2)، سابقه ی خانوادگی دیابت (3/1) و جنس مونث (2/1) از نظر آماری معنی داری بودند. اضافه کردن پروفایل های لیپید به مدل بالینی، سطح زیر منحنی (AUC) را فقط اندکی افزایش داد (5/72% در مقابل 4/71% و 001/0P=). مدل بالینی دیگری شامل دور کمر، قدرت پیش بینی کنندگی مشابهی با مدل بالینی شامل جنسیت مونث داشت. (سطح زیر نمودار 7/71% در مقابل 4/71% و 15/0P=).نتیجه گیریاین آنالیز نشان داد که در بزرگسالان تهرانی با FPG کمتر از 100 میلی گرم بر دسی لیتر، جنس مونث، سن بالا، سابقه ی خانوادگی دیابت، اضافه وزن یا چاقی و فشار خون بالا، به طور معنی داری با احتمال بالاتر وجود IGT ایزوله ارتباط دارند؛ بنابراین انجام آزمون تحمل گلوکز خوراکی در افراد دارای ویژگی های ذکر شده توصیه می شود.

صفحه 435مقدمههدف از مطالعه ی حاضر بررسی دقت تشخیصی تعریف های سندرم متابولیک طبق معیار های Modified ATP III و IDF در شناسایی افراد مبتلا به مقاومت به انسولین و ارزیابی میزان توافق بین این تعریف ها و مقاومت به انسولین است.مواد و روش هانمونه ها شامل 347 فرد غیردیابتی از مطالعه ی قند و لیپید تهران (140 مرد و 207 زن) با سن 84-21 سال بودند. مرجع استاندارد مقاومت به انسولین، HOMA-IR بود و مقاومت به انسولین به صورت 5/2 HOMA-IR ≥ در نظر گرفته شد.
یافته هابراساس معیار های ATP III و IDF به ترتیب 9/38% و 45% مبتلا به سندرم متابولیک بودند. حساسیت و ویژگی ATP III به ترتیب 3/52% و 65% و براساس IDF به ترتیب 60% و 59% بود. ضریب توافق کاپا بین ATP III و IDF با HOMA-IR به ترتیب 14/0 و 15/0 بود. دقت تشخیصی سطح زیر منحنی راک که از طریق شمارش تعداد اجزای سندرم متابولیک رسم شد، به نسبت پایین بود. وقتی که هر یک از اجزای سندرم متابولیک به عنوان مقادیر کمی در نظر گرفته شدند، دور کمر و قند خون ناشتا به تنهایی دقتی معادل یا حتی بیشتر از شمارش اجزای سندرم متابولیک داشتند.نتیجه گیریتعریف های Modified ATP III و IDF برای تشخیص مقاومت به انسولین از حساسیت کمی برخوردار بوده، توافق ناچیزی بین این معیارها و مقاومت به انسولین وجود دارد. اندازه گیری دور کمر یا قند خون ناشتا ساده تر است و برای تشخیص مقاومت به انسولین دقت بیشتری دارد.

صفحه 445مقدمهسندرم X یا سندرم مقاومت به انسولین یا سندرم متابولیک، مجموعه ای از عوامل خطرساز است که منجر به دیابت، قلبی عروقی و مرگ و میر ناشی از این بیماری ها می شود. با توجه به نقش مهم استرس اکسیداتیو در ایجاد عوارض این سندرم، هدف از این مطالعه بررسی اثر مکمل ویتامین E بر مقاومت به انسولین و سایر عوامل خطرساز در بیماران مبتلا به سندرم متابولیک بود.مواد و روش هادر یک مطالعه ی کارآزمایی بالینی و تصادفی دوسوکور، 70 بیمار مبتلا به سندرم متابولیک (45 زن و 25 مرد) با دامنه ی سنی 29 تا 57 سال، به طور تصادفی به دو گروه دریافت کننده ی 400 میلی گرم ویتامین E (35=n) و شاهد (دریافت کننده ی دارونما، 35= n) به مدت 3 ماه تقسیم شدند. پرسشنامه ی 24 ساعته ی خوراک در ابتدا و پایان ماه های اول، دوم و سوم برای هر بیمار تکمیل شد. مقادیر سرمی گلوکز (به روش آنزیماتیک)، انسولین (به روش رادیوایمونواسی)، لیپوپروتئین ها (به روش آنزیمی)، اسیداوریک و CRP (به روش رنگ سنجی) در شروع و پایان مطالعه اندازه گیری شد. برای آنالیز آماری از آزمون های تی، جفتی، آزمون مجذور خی و آنووا استفاده شد.یافته هاکاهش معنی داری در فشارخون سیستولی و دیاستولی در گروه مصرف کننده ی ویتامین E در پایان مطالعه در مقایسه با شروع مطالعه (به ترتیب 001/0=p و 009/0=p) و هم چنین تفاوت معنی دار فشارخون سیستولی در پایان مطالعه بین دو گروه وجود داشت (003/0=p). در گروه مصرف کننده ی ویتامین E، غلظت گلوکز و تری گلیسرید سرم نیز در پایان مطالعه در مقایسه با شروع مطالعه کاهش معنی داری داشت (به ترتیب 03/0=p و 01/0=p). به طوری که میزان گلوکز و تری گلیسرید در گروه مصرف کننده ی ویتامین E از 64/9±07/114 و 54/59±8/221 میلی گرم در دسی لیتر به ترتیب 7/9±05/101 و 77/56±65/197 میلی گرم در دسی لیتر کاهش یافت. کاهش معنی داری در گلوکز و تری گلیسرید سرم در پایان مطالعه بین دو گروه وجود داشت (به ترتیب 02/0= p و 04/0=p). اختلاف معنی داری در میزان انسولین سرم و مقاومت به انسولین در گروه مصرف کننده ی ویتامین E در پایان مطالعه در مقایسه با شروع مطالعه (به ترتیب 03/0=p و04/0=p) وجود داشت. میزان انسولین سرم از 6/1±3/8 میکروواحد در میلی لیتر در گروه مصرف کننده ی ویتامین E به 55/1±48/7 میکروواحد در میلی لیتر کاهش یافت.نتیجه گیریتجویز 400 میلی گرم ویتامین E به مدت 3 ماه در بیماران مبتلا به سندرم متابولیک باعث بهبود فشارخون، تری گلیسرید، گلوکز، انسولین و مقاومت به انسولین شد.

صفحه 455مقدمهمطالعه های مختلف درباره ی توانمندی افراد مبتلا به دیابت انجام شده است. با این وجود پژوهش های کمی درباره ی موانع و تسهیل کننده های توانمندی افراد مبتلا به دیابت در ایران وجود دارد. درک عوامل تاثیرگذار بر فرایند توانمندی، مراقبان بهداشتی را در تسهیل اثر بخش تر توانمندی افراد مبتلا به دیابت یاری می رساند.
مواد و روش هااین مطالعه یک تحقیق کیفی از نوع تئوری پایه است که با استفاده از مصاحبه ی عمیق بدون ساختار از 11 مرد و زن 73-21 ساله ی مبتلا به دیابت انجام شد. داده ها در فاصله ی بهمن 1385 تا تیر 1386 جمع آوری شدند و مضامین بر اساس مراحل ابتدایی روش اشترایوس و کوربین به دست آمدند.
یافته هاداده ها حاکی از آن بود که موانع توانمندی در بیماری دیابت مانند تنش، نگاه منفی به دیابت، سیستم های درمانی ناکارآمد و فقر و بی سوادی مشابه سایر بیماری های مزمن است. ضمن آن که آموزش دیابت، ترس از بیماری و عوارض آن و امید به آینده ای بهتر در تسهیل توانمندی افراد مبتلا به دیابت ضروری است. هم چنین مفاهیم پزشک؛ انسان مقدس، دیابت خواست خداوند، جسم هدیه ی الهی و حمایت همتایان و اعضای خانواده به خصوص دختران، از تسهیل کننده های اختصاصی در بستر اجتماعی و فرهنگی ایران بود.نتیجه گیریموانع و تسهیل کننده های توانمندی در افراد مبتلا به دیابت مشابه سایر کشور ها است. هم چنین، توانمندی در ایران به طور جدی تحت تاثیر باور های فرهنگی و مذهبی افراد مبتلا به دیابت قرار دارد. شناسایی این عوامل تاثیرگذار، می تواند مراقبان بهداشتی را در تسهیل اثر بخش تر فرایند توانمندی یاری کند.

صفحه 465مقدمهشواهدی وجود دارد که درمان کم کاری تیروئید با ترکیبی از T3 و T4 کارآتر از درمان فقط با T4 است. هدف از مطالعه ی حاضر مقایسه ی اثر درمان ترکیبی T3 و T4 با لووتیروکسین تنها بود.مواد و روش هااین پژوهش به روش کارآزمایی بالینی تصادفی و دو سو کور انجام شد. بیماران وارد شده در مطالعه 60-18 ساله بودند و دوز ثابت لووتیروکسین طی 3 ماه اخیر دریافت می کردند. بیماران، مطالعه را در مدت 4 ماه به پایان رساندند و به صورت تصادفی در دو گروه مساوی دریافت کننده ی درمان ترکیبی T3/T4 (25/6 میکروگرم لیوتیرونین 2 بار در روز به جای 50 میکروگرم لووتیروکسین دریافتی قبلی) و دریافت کننده ی T4 تنها تقسیم شدند. وزن، قد، فشارخون، تعداد نبض، سطح TSH، T4 تام، T3 تام، تری گلیسرید، کلسترول تام، LDLC و HDLC سرم اندازه گیری و پرسشنامه ی GHQ–28 در ابتدا و انتهای مطالعه اخذ شد.یافته هااز میان 71 شرکت کننده، 60 بیمار مطالعه را به پایان رساندند (در دو گروه 30 نفری). در گروه دریافت کننده ی درمان ترکیبی، سطح سرمی T4 تام کاهش و سطح سرمی T3 تام افزایش یافت (001/0P<). در حالی که مقادیر آن ها در گروه شاهد تغییری پیدا ننمود. سطح سرمی TSH در هر دو گروه قبل و بعد از مطالعه در محدوده ی طبیعی قرار داشت. در پایان مطالعه تفاوتی در وزن، ضربان قلب، فشار خون وهم چنین وضعیت چربی دو گروه درمانی دیده نشد. در مورد پرسشنامه ی GHQ–28 مجموع امتیاز ها بین دو گروه تفاوتی نداشت ولی در مورد زیرگروه مربوط به اضطراب و بی خوابی، درمان ترکیبی با T3/T4 باعث بدتر شدن امتیازهای کسب شده، گردید. (037/0=P).
نتیجه گیرینتایج این مطالعه نشان داد که درمان ترکیبی T3/T4 تاثیری در احساس خوب بودن، وضعیت سلامت عمومی، کیفیت زندگی و بهبود وضعیت پروفایل چربی بیماران مبتلا به هیپوتیروئیدی اولیه ندارد و درمان با لووتیروکسین به تنهایی، همچنان درمان استاندارد در این رابطه محسوب می شود.

صفحه 473مقدمهتاثیر هورمون های تیروئید بر مراحل مختلف تکامل سلول های مغز در مطالعه های مختلف نشان داده شده است. از طرفی تیروئیدیت پس از زایمان با تیتر بالای آنتی بادی ضد پراکسیداز در دوران بارداری همراه است. خودایمنی تیروئید در دوران بارداری منجر به کاهش T4 آزاد در سرم مادر با وجود عملکرد طبیعی تیروئید می گردد. این مطالعه با هدف بررسی تاثیر ابتلای مادر به تیروئیدیت پس از زایمان بر عملکرد تیروئید و تکامل شناختی کودک حاصل از آن بارداری طراحی شد.مواد و روش هااین مطالعه ی هم گروهی مشاهده ای از آبان 86 تا فروردین 1387 در کودکان متولد شده از مادران مبتلا به تیروئیدیت پس از زایمان در مقایسه با کودکان مادران با عملکرد طبیعی تیروئید در دوران بعد از زایمان انجام شد. نمونه ی خون و ادرار از کودکان و نمونه ی خون از مادران برای بررسی عملکرد تیروئید دریافت شد. تمام کودکان از نظر عملکرد شناختی به وسیله 7 آزمون روانشناختی توسط روانشناس بالینی مورد ارزیابی قرار گرفتند.
یافته ها124کودک، 55 نفر در گروه مورد (فرزند مادر مبتلا به تیروئیدیت پس از زایمان) و 69 کودک در گروه شاهد (فرزند مادر بدون ابتلا به تیروئیدیت پس از زایمان) ارزیابی شدند. متوسط سن کودکان در گروه مورد 4/1±5/9 و در گروه شاهد 1/0± 4/9 سال بود. هیچ تفاوتی از نظر عوامل زمینه ای احتمالی موثر بر بهره ی هوشی در دو گروه وجود نداشت. میانگین بهره ی هوشی کلی، عملکردی و کلامی در گروه مورد و شاهد به ترتیب 10±115 در مقابل 12±116، 11±113 در مقابل 12±116 و 12±113 در مقابل 13 ±113 بود. (5/0=P). از نظر سایر آزمون های عملکرد شناختی در هیچ یک از 7 آزمون روانشناختی تفاوتی بین دو گروه یافت نشد. هم چنین از نظر عملکرد تیروئید بین دو گروه تفاوتی دیده نشد.نتیجه گیریتیروئیدیت پس از زایمان در مادران سبب افزایش شیوع گواتر یا اختلال در عملکرد تیروئید، تکامل شناختی و بهره ی هوشی کودکان نمی شود.

صفحه 483مقدمهبیماری های گر های تیروئید شایع و یکی از روش های درمانی معمول آن جراحی است. یکی از عوارض شایع بعد از جراحی های تیروئید تشکیل کلوئید در قسمت قدامی گردن است که از نظر زیبایی مشکل بزرگی برای بیمار ایجاد می کند. گواتر ندولر از بیماری های شایع تیروئید است که درمان معمول آن جراحی است. با توجه به این که با ارزیابی صحیح بالینی و FNA خطر بدخیمی ندول های تیروئید کم است، لزوم یک روش درمانی غیر تهاجمی و بدون عارضه مطرح شده است.مواد و روش هااین مطالعه یک کارآزمایی بالینی از نوع مورد شاهدی است. 39 بیمار مونث مبتلا به گره منفرد جامد خوش خیم تیروئید اثبات شده توسط FNA وارد مطالعه شدند. 18 بیمار درمان شدند و 21 نفر نیز به عنوان گروه شاهد تا آخر مطالعه پیگیری شدند. درمان توسط لیزر فوتوکواگوگولیشن به صورت زیرپوستی با هدایت اولتراسونوگرافی (PLP) با استفاده از لیزر جراحی دایود با فیبر نوری 4/0 میلی متری و توان 3-1 وات انجام شد. بیماران قبل از شروع درمان و در فواصل 24 ساعت، 1 ماه و 3 ماه بعد از درمان معاینه شدند و حجم گره ها به وسیله ی اولتراسوند اندازه گیری شد. سپس میانگین حجم گره ها قبل و بعد از درمان تعیین و بین دو گروه مقایسه شد.یافته هامیانگین حجم گروه ها در گروه درمان شده با PLP بعد از 3 ماه از 23±3/26 سی سی به 13±1/15 سی سی کاهش یافت (001/0p<) اما در گروه شاهد از 97/22±67/27 سی سی به 24/23±81/27 سی سی رسید (5/0=P). میانگین کاهش حجم در گروه درمان شده 48/11±23/11 سی سی و میانگین افزایش حجم در گروه شاهد 03/1±13/0 سی سی بود (001/0 P<). به طور کلی میانگین کاهش حجم در گروه درمان شده 7/42 درصد و میانگین افزایش حجم در گروه شاهد 37/0 درصد بود. در بیماران درمان شده هیچ اسکار یا کلوئید در قسمت قدامی گردن بجا گذاشته نشد.
نتیجه گیریفوتوکواگولیشن با هدایت اولتراسونوگرافی می تواند به عنوان درمان جایگزین غیر جراحی به صورت حداقل تهاجمی و بدون عارضه برای گروه های خوش خیم دارای نشانه های بیماری تیروئید در بیماران نامناسب برای جراحی استفاده شود.

Page 423IntroductionTo determine the clinical traits of subjects with isolated impaired glucose tolerance (isolated-IGT) defined as fasting plasma glucose (FPG)<100 mg/dl and 2-hour plasma glucose (2h-PG) between 140 mg/dl and 200 mg/dl.Materials And MethodsIn this cross-sectional study, we evaluated the data and results of oral glucose tolerance test (OGTT) from 8212 participants of the Tehran Lipid and Glucose Study (4742 women and 3470 men) aged≥20 years, without diabetes. Logistic regression models for having isolated-IGT were developed among 7012 subjects with FPG<100 mg/dl to predict isolated-IGT.ResultsThe overall prevalence of IGT was 13.6% (n=1120) of these subjects, 59.6% (n=668) were isolated-IGT. In the clinical model, the adjusted odds ratios (OR) for having isolated-IGT were statistically significant for age≥40 years (2.6), hypertension (1.9), overweight (1.7) or obesity (2.2), family history of diabetes (1.3), and female gender (1.2). Adding the lipid profiles to the clinical model increased the area under the receiver-operating characteristics curve (AUC) only slightly (72.5% vs. 71.4%, P=0.001). The other clinical model including waist circumference had similar predictive power compared to the clinical model including female gender (AUC: 71.7 vs. 71.4, P=0.15).ConclusionThis analysis showed that in Tehranian adults with FPG<100 mg/dl, female gender, older age, family history of diabetes, overweight and obesity, and hypertension were significantly associated with a higher likelihood of isolated-IGT OGTT should hence be recommended in subjects with the above mentioned characteristics.Keywords: Isolated, impaired glucose tolerance, Diagnosis, Screening, Oral glucose tolerance test (OGTT), Prevalence

Page 435IntroductionThe aim of the present study was to assess the diagnostic accuracy of the modified ATP III and IDF definitions of the metablic syndrome in identifying of insulin resistant individuals by HOMA_IR and to assess levels of agreement between these definitions and insulin resistance.Materials And MethodsThe sample consisted of 347 nondiabetic subjects from the TLGS population (140 men and 207 women aged 27-87 years). The reference standard for insulin resistance was determined by HOMA-IR and insulin resistance was defined as HOMA-IR≥2.5.ResultsAccording to ATP III and IDF criteria 38.9%, 45% had metabolic syndrome respectively. The sensitivity and specificity of ATP III was 52.3% and 65% respectively, and this was 60% and 59% for IDF criteria respectively. Kappa agreement between modified ATP III and IDF with HOMA-IR was 0.14 and 0.15. Based on the area under the receiver operating characteristic curve constructed by counting metabolic syndrome components as recommended by modified ATP III and IDF diagnostic accuracy was fair. When each component of metabolic syndrome measures were considered as continous traits, waist circumference and fasting plasma glucose each separately had diagnostic accuracy equal or grater than at all the metabolic syndrome as a whole by ATP III and IDF criteria.ConclusionATP III and IDF definition, have low sensitivities for detecting insulin resistance and there is poor agreement between those criteria and insulin resistance. Page 445IntroductionSyndrome X or metabolic syndrome is a collection of risk factors which can lead to diabetes and cardiovascular diseases and result in death. Considering the important role of oxidative stress in causing the complications of this syndrome, the aim of this study was to investigate the effect of vitamin E supplementation on insulin resistance and associated risk factors in patients with metabolic syndrome.Materials And MethodsA randomized double blind placebo controlled clinical trial with parallel design was conducted on 70 metabolic syndrome patients, 29-57 years old, who were randomly divided into two groups one using 400 mg vitamin E (n=35), and group 2- given placebos (n=35) for 3 months 24 hour dietary recalls were completed in the beginning, and end of first, second and third months for each patient. Serum glucose and lipoprotein by enzymatic, Insulin by RIA, uric acid and CRP by colorimetric, insulin resistance by HOMA-IR methods were measured. For statistical analyses, student’s t-test, paired t-test, chi square and ANOVA were used.ResultsThere was significant difference in systolic and also diastolic blood pressure at the end of study in the vitamin E group compared to initial values (p=0.00, p=0.09 respectively). In this group, systolic blood pressure, serum glucose and triglycerides (TG) had significant decrease at the end of the study compared to the control (p= 0.003, p= 0.02, p= 0.04 respectively). Serum glucose, TG, insulin and insulin resistance had significant differences in the vitamin E group at the end of study compared to the beginning (p=0.03, p=0.01, p=0.03, p=0.04) Serum TG, glucose and insulin were 221.08±59.54, 114.07±9.64, and 8.3±1.6 at the beginning and 197.65±56.77, 101.05±9.7 and 7.48±1.55, respectively at the end of study.ConclusionIn metabolic syndrome patients, 400 mg vitamin E for 3 months had benetical effects on blood pressure, TG, glucose, insulin and insulin resistance.Keywords: Vitamin E Metabolic syndrome_Glucose_Insulin_Blood pressure_Cholesterol

Page 455IntroductionEmpowerment in individuals with people diabetes is well researched. However, few researchers have focused specifically on the barriers to and facilitators of empowerment in Iranians with diabetes. Understanding the barriers and facilitators that affect the empowerment process could help health professionals to more effectively facilitate self-empowerment.Materials And MethodsA grounded theory research design was used with in-depth interviews to collect the data from a purposive sample of males and females aged 21-73 years. Data were collected between February and July 2007. Themes were identified using the early steps of the grounded theory data analysis.ResultsCommon barriers to empowerment were similar to other chronic diseases viZ prolonged stress, negative view about diabetes, ineffective healthcare systems, poverty and illiteracy. Diabetes education, fear of diabete's complications, and hope for a better future also emerged as being crucial to empowerment. Facilitators specific to Iranians were the concept of the doctor as a holy man, accepting diabetes as God's will and the body as God's gift and support from peers and especially from daughters.ConclusionIranians with diabetes describe similar facilitators and barriers to empowerment quite similar to those as people from other countries as well as some specific to Iran. Empowerment is strongly affected by cultural and religious beliefs in Iran. Page 465IntroductionWe conducted this controlled trial to investigate whether combined therapy with levothyroxine and liothyronine in primary hypothyroidism, would lead to improvements in general health, quality of life, psychological problems and metabolic changes.Materials And MethodsThis study was a double blinded randomized control trial. Patients were invited to participate if they were between the ages of 18 and 60 years, and if they had been receiving treatment for primary hypothyroidism for at least 6 months, including a stable dose of levothyroxine for at least 3 months, and had normal serum thyroid hormone levels. Patients were randomized to receive combined levothyroxine plus liothyronine (patients in this group decreased their usual daily dose of levothyroxine by 50 μg and began taking 6.25 μg of liothyronine twice daily) or to continue their usual dose of levothyroxine. Treatment duration was 4 months. Weight, height, heart rate, blood pressure, total Cholesterol, LDL, HDL, Triglycerides, serum TSH, total T4, and total T3 levels were measured before and after of study. All patients completed the General Health Questionnaire -28 (GHQ-28) before and after the trial.ResultsSeventy-one participants were randomized into the 2 groups, and 60 completed the study. In the combined therapy group, serum T4 levels fell and serum T3 levels rose (P<.001), although both levels remained within normal limits. Serum TSH levels were similar in both groups at baseline and at 4 months. There were no differences between weight, heart rate, blood pressure and lipid profiles of two groups. For the GHQ-28, the overall score was similar but the anxiety/insomnia subscale was significantly higher (indicating worse psychological well-being) for combined treatment compared with T4 alone (P=0.037).ConclusionThe only biochemical change was the expected rise in serum T3 levels and fall in serum T4 in the combined therapy group, because the blood samples were drawn only 1-2 hour after the morning dose of liothyronine. In conclusion, we found no evidence that combined T4/T3 replacement, resulted in improved well-being, general health, quality of life and lipid profile compared with T4 alone. Page 473IntroductionThe effect of thyroid hormones on all stages of the development of central nervous system has been documented in different studies. Postpartum thyroiditis (PPT) is associated with high titers of TPO-Ab during pregnancy. Thyroid autoimmunity in pregnancy results in reduction of serum free thyroxin level, even in the presence of normal thyroid function. This study was conducted to evaluate the effect of PPT on the thyroid function and cognitive development of the offspring.Materials And MethodsIn this prospective cohort study, conducted between December 2007 and March 2008, children born to mothers who had developed postpartum thyroiditis after delivery and those mothers who had not, were included in this study. Blood and urine samples were obtained from all children and blood samples were drawn from their mothers. All children underwent 7 cognitive tests in a clinical psychology laboratory.ResultsWe enrolled 124 children for the study, 55 cases (children of mothers with PPT) and 69 controls (children of mothers without PPT). Average age of cases was 9.5±1.4 and in controls was 9.4±0.1 (P=0.2). No difference was noted between the two groups for baseline characteristics which might have affected their neurodevelopment. Mean scores for total IQ, performance IQ, and verbal IQ in cases and controls were as follows: 115± 10 vs. 116±12, 113±11 vs. 116±12, and 113±12 vs. 113±13, respectively (P=0.5). There was no difference between the results of the other tests (CPT, WCST, PRM Test, SWM Test, MFVP Test, SOC) in the two groups. No difference was noted between the thyroid function of the two groups.ConclusionPrevalence of goiter, thyroid function, cognitive development, and intelligence quotient of children born to mothers who later develop postpartum thyroiditis are similar to those of normal children. Page 483IntroductionThyroid nodules are common and surgery is the usual treatment. One of the common complications after thyroid surgery is keloid formation in front of the neck which is an important cosmetic problem. Nodular goiter is a common disorder of thyroid and its common treatment is surgery. Considering the fact that with clinical evaluation and FNA, the risk of malignancy in these cases is low a non aggressive approach to this complication is necessary. This study aims to evaluate of the effectiveness of percutaneous laser photocoagulation (PLP) in reducing the volume of benign thyroid nodules.Materials And MethodsIn this case-controlled clinical trial, 39 female euthyroid patients with solitary solid benign thyroid nodules were enrolled 18 patients were treated with ultrasound guided percutaneous laser photocoagulation, and 21 patients were included in the control group. PLP was performed by diode surgical laser with 1-3 watt output for 123-1019 seconds. All patients were followed up for 3 months.ResultsAfter PLP, mean nodule volume decreased from a baseline value of 26.3 ± 23 mL to 15.1 ± 13 at 3 months. Mean nodule volume reduction in comparison with baseline was 42.72%. Compressive symptoms showed relative decrease. No serious complication was seen, nor was any keloid or scar observed in the front of the neck.ConclusionPLP can be a minimally invasive effective alternative treatment for symptomatic benign solitary solid thyroid nodules in patients not suitable for surgery.Keywords: Thyroid, Nodule, Laser
